With the form designer, you can create more customised entry forms with additional subtitles, text elements, and separating lines than in the default entry form, which is created only based on the variables. 

This feature can be useful in cases where multiple participants are to register data, and more information on how to fill in the data is needed. You can also create multiple forms on the same entry, i.e., in cases where different research collaborators are to fill out different parts of the dataset, or data on the same subject/entry shall be entered at different time points. 

Please note: Variables that are not included in a form will not be available for registration when forms are set active. 

  1. Click "Project design" and select "Forms"

  2. Click "Create form"

  3. Give your new form a name and click "Create" If you want to create a form for a series, check the "Create this form based on a specific series" box (See more about Series in forms) 

  4. Click on the form card to open it

  5. Now you are ready to start building your form. The different modules used to construct a form are shown on the left while you compose your form on the right side.

  6. Fill out the form's title at the top of the page (optional)

  7. Drag and drop elements from the left side into your form designer, or click on the "+" icon on the element card (which appears when hovering over it). The elements available are:

    1. Form elements

      • Subtitles: Add the subtitle element to your form and click on it to fill in the field with your subtitle

      • Text: Add the text element to your form and click on it to fill in the text field with your text

      • Separators: These are thin lines that can be used to separate different parts of your form. Add the separator element to your form, and a line will appear.

    2. Variables:

      • Add variables to your form and place them in the desired order. If you have assigned variables into groups or series in your variable setup, you can add entire groups of variables or series into your form.NB: If you use the search function to find variables and variable groups, be aware that only variables within a group that contains the search text in its name are included and can be moved to the form (i.e. groups may appear empty if you use a search term that is only included in the group's name and not in the names of the variables inside it).   

  8. Edit the variable name and appearance if wanted (See "Edit form variables")

  9. All elements can be reordered in your form. Drag and drop the elements in your preferred order.

  10. To preview how your form will appear, click the eye icon in the upper-right corner. NB: If you want the form to be used for data entries right away, activate the "Activate form" toggle switch in the upper right corner. If this is not activated, your form will be saved but not set for use in data entries. To be able to activate a form, all mandatory variables MUST be included.   

  11. When you are done creating your form, click "Update form" in the upper-right corner. 

  12. Click "Done"
